COMMUNITY HEALTH and DEVELOPMENT PROGRAMS, SERVICES, AND INTERVENTIONS ARE OFFERED TO PROMOTE HEALTHY LIVING TO PREVENT AND/OR BETTER MANAGE DIET RELATED CHRONIC DISEASES, AND MOST IMPORTANTLY, ADDRESS THE SOCIAL RISK FACTORS THAT IMPACT HEALTH OUTCOMES. WE ORGANIZE, EDUCATE AND EMPOWER STAKEHOLDERS TO WORK COLLECTIVELY TO CHANGE POLICIES, SYSTEMS, AND ENVIRONMENTS THAT ADVERSELY AFFECT BLACK, LATINO AND INDIGENOUS PEOPLE OF COLOR FROM BEING AS HEALTHY AS POSSIBLE. ECONOMIC DEVELOPMENT CREATES HEALTHY RESILIENT COMMUNITIES, PROVIDING LOW TO MODERATE INCOME AND AFFORDABLE HOUSING AND COMMERCIAL SPACES, AS WELL AS OPEN GREEN SPACES. YOUTH DEVELOPMENT FOCUSES WORKFORCE READINESS, SUMMER EMPLOYMENT, AND COLLEGE AND CAREER EXPLORATION TO PREPARE YOUTH FOR SUCCESS.
